The VA in DC and at Perry Point have approved adult day care providers to invoice for their registered Veterans if the center does the following:
- The center nurse must speak with the Veteran regarding medical concerns with nursing documentation (see attached form).
- On the same day, another staff member must call and speak with the Veteran, as well as document food delivery on the Daily Care Connection Form
Very Important: For the center to bill the VA, 2 calls must be made that day to the Veteran.
Along with the UB04, the completed forms must be included as well as the menu or proof of what food was provided. Invoices will not be reviewed for payment without all of the supporting documentation.

Submit DC invoices to Drucilla Davis.
